Transitions connects community services and healthcare providers, recognizing the importance of well-timed quality care for individuals with a life limiting illness.
It’s difficult living with a life-limiting illness. Transitions supports clients and families as they face medical challenges and other life changes. We help clients to identify needs and obtain assistance as necessary. By encouraging clients to define what “living well” means to them, Transitions enables clients and their loved ones to retain a sense of control over their lives, even in the face of a progressive illness.
Transitions services are available at no charge and are generously provided through the support of our volunteer division.
Transitions offers customized services that address the specific needs and goals of each client. Clients can receive one-time or ongoing phone support as well as visits to their home or place of residence. Services are provided by the Transitions Program Coordinator and trained volunteers.
Any adult with a progressive life-limiting illness may be eligible for Transitions. This includes those who currently receive other in-home services (such as respite or home health). Prospective clients and their physicians consult with the Transitions Program Coordinator to make a determination of eligibility.
