Our comprehensive care assessment and follow up with a professional Geriatric Care Manager (GCM) will assist you in creating a plan to face the early stages of disease to end-of-life care. Our Geriatric Care Managers are here 24 hours a day, 365 days a year, to offer long-term planning, regular communication with families, placement and decision-making assistance, counseling and family intervention, crisis management and resolution, service plan oversight and quality assurance.
